If your organization or ministry is serving the homeless and low-income individuals and families in Oklahoma, we want to hear from you.

The purpose of this survey is to:

Learn about your organization

Find out what your organization is currently offering the community

Estimate how many people your organization helps and where

Figure out what your organization needs and find ways to help

Discover the current challenges and needs of homeless individuals and families

Reveal gaps and barriers in services provided

Identify problems and/or limitations in current services currently being offered
