Transition House, Inc. operates a Transitional Living Program (TLP) offering supervised apartment living, intensive case management, daily psychoeducational groups, transportation to basic needs, and a Community Outreach Program (COP) including a Drop-In Center with social/recreational activities and support. The TLP is a temporary supportive housing program intended to help adults with serious mental illness transition from high-acuity settings (including inpatient care, homelessness, extreme poverty, or legal crises) into community living.

- Hours / availability
- Transition House is active and operating. The Drop-In Center is located at 700 Asp, Ste. 4 and is available for client use on weekdays during business hours (brochure/agency materials indicate roughly 9:00 AM–5:00 PM weekdays; clients may use the center after hours with staff approval and by checking out a key). The TLP length of stay is typically less than one year (up to ~12 months); services include weekly one-on-one meetings with Recovery Coordinators, daily groups, and scheduled grocery/transportation support. Admissions/screening statistics and board meeting activity indicate ongoing operations through 2026.
- Eligibility
- Program is for adults with serious mental illness; the TLP serves people transitioning from high-acuity situations (e.g., inpatient treatment, homelessness, extreme poverty, legal issues). The organization also serves people with co-occurring substance use disorders. Applicants are screened by staff prior to acceptance. Clients must provide proof of income when applicable (pay stubs or Social Security award letters) as part of fee determination.

- What to bring
- Suggested/prescribed items per Client Handbook: clothing, personal hygiene items, bedding, books/magazines, alarm clock, journal, photos, personal laptop/tablet (for program use), and any documentation required for intake (photo ID, proof of income such as pay stubs or Social Security award letters). Note: all personal items are screened by staff; certain items (weapons, illicit drugs/alcohol, large electronics, unapproved wood furniture, etc.) are prohibited.
- Cost
- Client fees (per the organization's Client Handbook): clients with an income are charged 35% of the previous month's income; clients with no income are expected to pay a sliding minimum fee: $50 (months 1–3), $65 (months 4–6), $80 (months 7–9), $95 (months 10–12). Program extension clients add $15 to the previous minimum every three months. (The organization's internal program cost per 'transitional living day' is reported in board materials as $83/day.)
